Privacy officer

In accordance with Quebec's Act respecting the protection of personal information in the private sector, Dr. Isabelle Tremblay, founder, acts as the clinic's person in charge of the protection of personal information.

Information we collect

Identity and contact details, insurance information, medical and dental history, X-rays, clinical photos, treatment plans and billing history.

We collect only the information needed to provide safe, high-quality care.

Consent

We collect, use and disclose your information with your clear, free and informed consent. You may withdraw your consent at any time, subject to the clinic's legal and professional obligations.

Disclosure to third parties

With your consent, your information may be shared with your insurance provider, a dental laboratory or a health professional to whom you are referred.

We never share your information for commercial purposes.

Retention and destruction

Records are kept for the period required by the Ordre des dentistes du Québec, then securely destroyed.

Your rights

You may request access to your information, have it corrected, or withdraw your consent. We respond to any written request within 30 days.

Confidentiality incidents

In the event of an incident posing a risk of serious injury, we will notify the affected individuals and the Commission d'accès à l'information, and keep a register of incidents.

Any complaint may be sent to our privacy officer. If you are not satisfied with our response, you may contact the Commission d'accès à l'information du Québec.
